Medical Accommodation

Effective September 6, 2010 students seeking academic
accommodation should request a UWO Student Medical Certificate be
filled out at initial visit with physician. The fee is $20 and is
due at time of request. |

- Form for off-campus physicians NOTE: Type in student number, name, and print the form for the physician to complete. Bring completed form to the Academic Counselling Office, SSC 2105.
